Notarizing Consent and Waiver Forms for LA Events and Schools

Schools, youth programs, sports leagues and event organizers collect a lot of consent and waiver forms. Most never need a notary. A few do, because a venue, insurer, agency or program asks for it. If you are in Los Angeles and think some of your forms fall into that group, here is how to plan.

First question: who is asking for notarization?

Do not assume. A notary cannot tell you whether a form should be notarized, and we cannot give legal advice about what it should say. Ask the party that requires the form, whether that is an agency, an insurer, a venue or your organization's attorney. Requirements vary, and getting a clear answer early prevents a wasted trip.

Think about who the signers are

Different signers raise different questions:

  • Adults signing for themselves: straightforward, with valid government-issued photo ID.

  • Parents or guardians signing for someone else: the requesting party may have specific rules about who may sign and what proof it needs. Ask them before the appointment.

  • People who are not available in person: a notary cannot notarize unless the signer is physically present, so plan a time when everyone who must sign can attend.

Preparing the forms

  1. Finalize the wording before the signing. Do not change a form after it has been signed.

  2. Print complete copies with every field filled in, other than the signature and any items the signer must complete personally in front of the notary.

  3. Keep a roster with names, forms and contact details.

  4. Make spares, since a spoiled page can mean a reprint.

  5. Remind signers to bring valid ID, and to leave signature lines blank.

Running the signing

For a large group, choose a location with space for a check-in table and a signing table. Give people arrival windows so the line does not form all at once. We identify each signer, complete the notarial act and record every act in our journal. Because each signature gets its own entry, a large group takes planned time. Large-volume signings are a core service for us.

Timing that suits families and staff

Parents and volunteers often work during the day. We offer overnight and after-hours appointments, including evenings, nights and weekends when available, which can make it easier to reach everyone in one session.

After the signing

Compare your roster against the signed forms, store originals as your organization requires and follow up with anyone who could not attend.

If you are organizing an event in LA, tell us the location, the number of signers and the forms involved, and we will plan with you.
